NITRO, W.Va. (AP) — Federal regulators have cited a chemical company for a dozen violations at a plant in West Virginia where a worker was killed in June.

The U.S. Occupational Safety and Health Administration announced the citations against AC&S Inc. on Thursday.

OSHA says the worker died while sandlblasting at the plant in Nitro, W.Va. Federal investigators found that the air line for the worker's air hood was hooked up to a nitrogen gas line.

The alleged violations include failing to properly label gas systems and failing to provide training on hazardous chemicals.

OSHA says all 12 violations are serious.

The company says it began immediately following the accident to work with OSHA to address the agency's concerns.

AC&S says it wants to ensure that such an accident doesn't happen again.
