The husband of a Sydney woman stabbed to death by her brother, Antony Waterlow, says the case has identified systemic failures in the mental health system.

An inquest in investigating the deaths of Chloe Heuston and her father, art curator Nick Waterlow, in Sydney's eastern suburbs in November 2009.

Antony Waterlow was later found no guilty of their stabbing murders, due to his untreated paranoid schizophrenia.

The coroner has previously heard of repeated attempts by family and friends to have Antony Waterlow scheduled and forced into treatment for his mental illness.

In a letter read to the court today, Chloe Heuston's husband, Ben Heuston, said Anthony Waterlow's behaviour was terrifying but it seemed he had to be caught actually hurting someone before the system would act.

Mr Heuston says it should be mandatory for the views of family to be taken into account when a mentally ill patient is considered for scheduling.
