An investigation into the death of a sailor, who fell off a container ship off Perth, has found safety procedures were not followed.

The 27-year-old Filipino man was swept overboard by a wave while working on a ladder on the MSC Siena near Rottnest Island in 2011.

The container ship was approaching Fremantle Harbour at the time.

The Australian Transport Safety Bureau has found a risk assessment for the task was not carried out, and the poor weather conditions were not taken into account.

It says the ship's manager, Allseas Marine, has since taken steps to improve training and revise its polices for working on ladders.
