A 54-year-old man has changed his plea midway through his trial on a charge of causing grievous bodily harm to another man in a road rage attack.

Robert Wilson Shepherd had originally maintained he had acted in self defence when he punched 63-year-old Ali Hussain on the side of the Kwinana Freeway in November 2011.

Dr Hussain testified he was knocked unconscious and suffered a fractured nose and cheekbone.

The doctor also said he received cuts to his face.

He said the attack occurred after he had pulled Shepherd over to discuss a minor collision.

Shepherd's lawyer told the court his client maintained he acted in self defence because Dr Hussain pursued him.

Witnesses later told the court they saw the doctor being repeatedly punched and kicked.

After the evidence, Shepherd changed his plea to guilty.

He was remanded in custody and will be sentenced in March.
